The CHAMP study addresses three important objectives including:

  • Opioid Use Disorder (OUD) Detection – Can OUD screening be effectively incorporated into primary care mental health screening protocols?
  • Collaborative Care (CoCM) Effectiveness – Does implementing Collaborative Care for OUD and mental health disorders improve outcomes?
  • Sustainment – What implementation strategies are effective at sustaining Collaborative Care programs that concurrently manage mental health disorders and OUD?

This trial will be conducted in primary care settings with existing CoCM programs or a commitment to initiate a CoCM program in the next 6 months. 

CHAMP Study Design

All participating clinics will begin systematic screening for OUD using a modified and shortened version of the NIDA-Modified ASSIST screener.

Clinics will complete a Fidelity Assessment for CoCM. Based on this assessment:

  • High fidelity clinics receive 3 months of preparation and training (July 2020 – October 2020) with screening and recruitment initiated November 2020.
  • Low fidelity clinics receive 9 months of preparation and training (July 2020 – March 2021)  with screening and recruitment initiated April 2021.

Participating clinics will be randomized to provide CoCM for mental health conditions and OUD (Intervention) or CoCM for mental health conditions only (Control).

All participating clinics will identify, recruit, and enroll patients with co-occurring disorders into the study.

Intervention: CoCM for Mental Health Conditions and OUD (CoCM for Co-occurring Disorders)

Members of the CoCM team (care managers, primary care providers, psychiatric consultants) will participate in CHAMP trainings conducted by the AIMS Center. CoCM teams will participate in the implementation of, or a refresher training in CoCM for treatment of mental health disorders and treatment for OUD. Primary care providers and psychiatric consultants will also participate in trainings to become waivered to deliver Medication Assisted Treatment (MAT) for OUD.

At the end of the trial, clinics will receive assistance from the AIMS Center to sustain their CHAMP Intervention program.

Control: CoCM for Mental Health Conditions

Members of the CoCM team (care managers, primary care providers, psychiatric consultants) will participate in the implementation of a new CoCM team or refresher training in CoCM for mental health disorders conducted by the AIMS Center. Although clinics randomized to this condition will not receive specific training in treating OUD, they will be completely free to treat OUD consistent with their current treatment options (usually referral )

At the end of the trial, clinics will have the opportunity to participate in CHAMP Intervention trainings conducted by the AIMS Center, along with ongoing implementation support.
